Job summary

Evolve Therapy Services, LLC seeks an Occupational Therapy Assistant for an in-house Skilled Nursing Facility in sunny South Florida. This is a full-time, contract-to-permanent role with relocation assistance and a flexible schedule. Part-time or PRN options may also be available, offering growth within our team.

You will plan, organize, develop, and direct Occupational Therapy Services to ensure the highest quality resident care while adhering to federal, state, and local standards.

Qualifications

• Associate's degree in Occupational Therapy.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatment plans and follow short- and long-term goals per care plan.
• Interpret and communicate evaluation findings and recommendations to residents and families.
• Establish individual resident therapy schedules with team members.
• Deliver therapy per treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily.
