Job summary

TalentBurst is hiring an RN Case Manager for an on-site 13-week contract starting 9/28/2026. The role includes case management for inpatient and observation patients, coordinating discharge planning with the care team, and ensuring InterQual-based progression of care. TX-licensed Social Worker credentials are valued for collaboration with the team.

Qualifications

List typical procedures performed on unit(s)

• Case Manager: Provides Case management service for both inpatient and observation patients assigned. Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team. Evaluates progression of care using InterQual throughout the episode of care.
• Social Worker: Works collaboratively with RN case managers and the multidisciplinary team, performs assessments and develops appropriate discharge plans for high‑risk patients with discharge planning or psychosocial needs. Engages in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.
