Position Details

- Shift - 8hrs Days (08:00 am - 16:30 pm)
- Floating: Possible to any unit
- Orientation: Will vary based on skillset/comfort level – 2-5 days departmental
- Scheduling:
- Weekend rotation: no weekends
- No Call
- Holiday Expectations: Travelers will work all holidays unless approved off in contract
- RTO: Manager approval
- Shift times: Will vary based on requisition
- RN: 8h shifts – will work 5 8's with an 0800 start

- Bariatric Center of Excellence
- Department Information:
- Average census and case load/day - 99% capacity on most days; 14-16 patients – dependent on unit assigned
- Minimum Years of Experience required - At least 3-5 years in Acute Care Case Management – please decline if this is not their most recent experience
- Will you accept a first-time traveler - No, must have traveled as an RN-Case Manager
- Patient Population:
- Dependent on unit – Adult Medical/Surgical, Cardiac, Ortho and possibly Peds
- Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:
- RN: BSN required; BLS; Case Management certification (preferred)
- Skills required:
- RN:
- EPIC experience is required
- Skilled in placement for SNF/IPR/LTAC/Hospice/Home Health, DME, Behavioral Health needs/DSS (peds), Difficult complex placement, will work routine with homeless population
- Care management skills: assessment, care coordination and discharge planning
- Support within the Department:
- Director
- Manager(s)
- CMO
- UM team that is very supportive – PA is part of this team
- Peer CM's to support
- Social Worker(s)
- Technology/Equipment:
- EMR: EPIC – required
- Rover Chat in EPIC and will carry a phone
- Careport – referral management (similar to Allscripts) – only preferred experience
