Job summary

Spectrum Health Systems in Lowell, MA seeks a Continuing Care Coordinator to connect clients leaving the Inpatient Residential Services program with community resources and aftercare counseling on campus.

You will perform case management for housing, job searches, medical and social services, and coordinate referrals with community providers while supporting a smooth transition to continuing care and tracking program outcomes.

Qualifications

• LADC I or CADAC certification preferred but not required.
• At least one year experience in substance abuse treatment or closely related field.
• Strong documentation, organizational, and communication skills.
• Must have basic ability in use of computers to enter entries and utilize electronic health records.
• Minimum of a high school diploma or equivalent.

Responsibilities

• Coordinate continuing care recovery services for clients returning to the community after completion of the Inpatient Residential Services program and provide aftercare counseling on campus.
• Perform case management activities based on clients' continuing care plans regarding housing, job searches, and medical, social, education, and support services.
• Coordinate continuing care referrals by developing relationships with community providers and scheduling initial appointments.
• Work closely with Clinical staff to ensure smooth transition from residential to continuing care program.
• Collect data and report program success rates as directed.

TheContinuing Care Coordinatoris responsible for:

• The coordination of continuing recovery services for clients returning to the community after completion of the Inpatient Residential Services program and for providing aftercare counseling services to clients on campus.
• Performing case management activities based on clients' continuing care plans regarding initial housing, job searches, and establishment of medical services, social services, education, support services, and services from other community providers.
• Coordinating continuing care recovery referrals by developing positive relationships with community providers and including scheduling of initial appointments.
• Working closely with the Clinical staff in the Inpatient Residential Services program to ensure the clients transition smoothly from the residential portion of treatment to the continuing care program.
• Collecting data and program success rates as directed.
