Job summary

Spectrum Health Systems, Inc. in Lowell, MA seeks a Continuing Care Coordinator to support clients transitioning from Inpatient Residential Services to community-based continuing care.

You will coordinate ongoing services and provide aftercare counseling on campus to ensure smooth, sustained recovery. The role requires strong case management, documentation, and communication skills, with preferred credentials (LADC I/CADAC) and at least one year in substance abuse treatment or related field.

Qualifications

• LADC I or CADAC certification preferred but not required.
• Minimum of one year experience in substance abuse treatment or case management.
• Strong documentation, organizational, and communication skills.
• Basic computer skills to enter data and use electronic health records.

Responsibilities

• Coordinate continuing recovery services for clients returning to the community after completion of the Inpatient Residential Services program.
• Provide aftercare counseling to clients on campus to support ongoing recovery.
• Perform case management related to housing, employment, medical services, social services, and community referrals.
• Develop positive relationships with community providers to coordinate referrals and initial appointments.
• Collaborate with clinical staff to ensure smooth transition from residential to continuing care.
