Job summary

Cheer Home Care is actively seeking Private Duty Licensed Vocational Nurses to deliver high-quality skilled nursing care for the elderly and disabled in a private duty setting.

As an LVN on the 12-hour NOC shift, you will coordinate with the care team, educate clients on diagnoses and treatments, and provide wound care, medication management, and chronic condition management with a focus on compassionate, patient-centered care.

Qualifications

• Current, unrestricted California LVN license.
• Graduation from accredited practical or vocational nursing program.
• 1 year of LVN experience.
• Home Health experience preferred.
• Proof of negative TB result.
• Physical screening documentation within the last year.
• Current BLS certification.
• Valid California driver’s license and insurance.
• Cell phone with internet access.

Responsibilities

• Coordinate with physicians, rehab services, and other care providers.
• Educate client on diagnosis and treatment, ensuring understanding.
• Provide skilled nursing services including wound care, medication management, and managing chronic conditions.
• Highly organized and maintain accurate documentation of care.
