Cheer Health Care is actively hiring per diem Licensed Vocational Nurses (LVN) for private duty home care in La Jolla, San Diego North County. The role offers a competitive pay rate of $35.00–$50.00 per hour, with 12-hour day shifts on Monday, Wednesday, and Saturday.

You will deliver premium home-based nursing care to seniors and adults with complex needs, collaborate with physicians and family members, and ensure precise, timely documentation of patient progress.

Qualifications

• Current, unrestricted California LVN license.
• Graduation from accredited practical or vocational nursing program.
• 1 year of experience as a Licensed Vocational Nurse.
• Home Health experience preferred.
• Proof of Negative TB result.
• Physical Screening Documentation within the last year.
• Current BLS certification.
• Valid California driver's License and insurance.
• Cell phone with internet access.

Responsibilities

• Advanced Clinical Management: administer medications, wound care, monitor chronic conditions.
• Interdisciplinary Collaboration: coordinate with physicians, PTs, and families.
• Family & Patient Empowerment: educate clients and caregivers on health updates and care plans.
• Accurate Clinical Reporting: document client progress and care provided.
