Job summary

Cheer Home Care in California seeks a dedicated LVN to provide skilled nursing care to seniors and adults with disabilities. You will coordinate with physicians, educate clients about diagnoses, and document care accurately in a collaborative team.

Qualifications

• Current, unrestricted California LVN license.
• Graduation from accredited practical or vocational nursing program.
• 1 year of experience as a Licensed Vocational Nurse.
• Home Health experience preferred.
• Proof of Negative TB result; Physical screening within the last year.
• Current BLS certification.
• Valid California driver's license and insurance.
• Cell phone with internet access.

Responsibilities

• Coordinate with physicians, rehab services, and other care providers.
• Educate on client’s diagnosis and treatment, ensuring understanding.
• Provide skilled nursing services including wound care, medication management, and managing chronic conditions.
• Maintain high organization in providing and documenting care.
