Responsibilities

• Participates in and maintains awareness of performance improvement initiatives.
• Conducts accurate clinical assessments and documents findings.
• Develops, implements, and evaluates individualized care plans.
• Provides direct nursing care per physician orders in a complex care setting.
• Mentors/precepts junior staff and supports department staff development.
• Ensures discharge planning is appropriate and resources are referenced.
• Collaborates with interdisciplinary team to optimize patient outcomes.
