Qualifications

• Ability to effectively communicate with patients, families, physicians, and healthcare team.
• Knowledge of and application of the nursing process.

Responsibilities

• Participates in performance improvement initiatives.
• Evaluates and integrates evidence-based practice into clinical practice.
• Conducts and documents accurate clinical assessments.
• Develops, implements, and evaluates the patient’s individualized plan of care.
• Uses nursing judgment to tailor the plan of care to patient needs.
• Manages patient assignment and delegates tasks appropriately.
• Develops an educational plan for patient and family and documents per standards.
• Supports patient/family rights and informed decisions.
• Continuously revises the care plan as health status changes.
• Develops discharge plans with referrals to community resources.
• Supports staff development via precepting and training.
• Participates in departmental committees as needed.
