Job summary

This Per Diem role in Newton, MA requires an ASN (BSN preferred) and an active MA/NH RN license, with 2+ years in case management or discharge planning desirable; must be available for at least two 8-hour days per month.

Qualifications

• Active RN license in MA and NH.

Responsibilities

• Coordinate and support healthcare within the facility
• Provide outreach and enrollment services to meet eligibility requirements
• Modify patient treatment plans based on responses and conditions
• Prepare patient/family for discharge
• Communicate with third-party payers for service authorization
• Review challenging cases with medical directors as needed
• Define care goals by providing education and direction to patients and families
• Maintain accurate and detailed reports and records
