Job summary

TGH Senior Center Powered by Greenbrook Medical in the Greater Tampa Bay Area is seeking a Complex Care Manager. You will own the clinical coordination for our most medically complex seniors, guiding them from hospital admission through recovery back to the community.

As part of the care management team, you’ll collaborate with physicians, center managers, pharmacists, hospitalists, SNFs, and community partners to prevent unnecessary ER visits, ensure timely follow-up, and continuously improve

Qualifications

• Active RN or LPN license.
• 3+ years in nursing, care management, or complex care coordination.
• Experience with managed care, Medicare Advantage, or value-based primary care.
• Strong clinical judgment and triage skills.

Responsibilities

• Coordinate care for high-risk patients across hospitals, SNFs, and clinics.
• Manage transitions of care and ensure timely follow-ups.
• Collaborate with physicians and care teams to improve outcomes.
• Use data to identify gaps and drive improvement.
