Cornell Scott–Hill Health Center in Connecticut is seeking a Care Coordinator to join our Care Management team. You will coordinate referrals, support care plans, and connect patients with community resources, while collaborating with medical and behavioral health providers.

The role emphasizes strong communication, organization, and problem-solving. Applicants should have a high school diploma; an associate or bachelor’s degree and CHW certification are preferred; experience with vulnerable

Qualifications

• High School Diploma/GED required; Associate or bachelor’s Degree preferred.
• Care Management/CHW certification preferred for designated roles.
• Experience in health/social services is preferred.
• Knowledge of New Haven community social/health services is preferred.
• At least 1 year experience with relevant population.
• Computer skills including data processing and e-mail are required.

Responsibilities

• Interacts with patients and caregivers to ensure continuity of care and adherence to care plans; identify barriers to adherence.
• Coordinates care with care team to overcome financial or social barriers to medical care.
• Provides care coordination/management services to individuals, groups, or families in CS-HHC setting or community, per professional standards.
• Assists in obtaining insurance, entitlements, and other authorizations for health and behavioral health services.
• Identifies client/patient need for advocacy in accessing health systems and community resources.
• Performs other duties as assigned by the Director of Care Coordination.
