Cornell Scott–Hill Health Center is seeking a dedicated Care Coordinator to join the Complex Care Management team in Ansonia, CT. The role collaborates with medical and behavioral health providers to coordinate comprehensive, patient-centered care.

The ideal candidate will have strong communication, organizational, and problem-solving skills, with experience in care coordination, data management, and working with diverse populations.

Qualifikationen

• High School Diploma/GED required; Associate or bachelor’s Degree preferred.
• Care Management, CHW Certification(s) preferred and/or required for designated roles.
• Human Service/Health Care experience is preferred.
• At least one year experience with relevant population.
• Computer skills, including data processing and e-mail abilities are required.
• Excellent health care support and organizational skills required.
• Knowledge of professional ethics essential.
• Ability to work independently or with supervision.

Aufgaben

• Interacts with patients and caregivers to ensure continuity of care and adherence to care plans.
• Coordinates care with care team to overcome financial or social barriers to medical care.
• Provides care coordination/management services to individuals, groups, or families.
• Assists with obtaining insurance, entitlements, and other resource authorizations.
• Identifies client needs for advocacy in accessing health systems and community resources.
• Performs other duties as assigned by the Director of Care Coordination.
