Job summary

Qualifications

• BSN preferred
• CCM or equivalent certification
• Three years or more of related experience and/or training or equivalent combination of education and experience

Responsibilities

• Identify, assess, plan, coordinate, implement, monitor, and evaluate client care using the client's outcome management model.
• Obtain written authorization for case management services and release of information.
• Coordinate with family, physician, and health care team to develop a plan of care.
• Identify and secure services and resources to implement the plan.
• Monitor services for continued appropriateness and report to client and payer.
• Provide ongoing instruction to client and family regarding injury/disease process and treatment options.
• Travel to patient appointments as needed.
