SCM Associates, Inc. seeks an experienced Case Manager (RN) to assess, plan, coordinate, and monitor care for clients in a home-based setting. You will collaborate with families, physicians, and the care team to ensure quality, cost-effective outcomes and adherence to care plans.

Travel to patient appointments is required several times per week. Applicants should hold an RN license with CCM or equivalent certification preferred and have three years of related experience.

• Identify and assess client needs; plan, coordinate and monitor case management services
• Collaborate with client, family, physician and care team
• Coordinate services and resources to implement the plan
• Monitor progress and adjust care as needed
• Provide instruction on care options and outcomes
• Travel to patient appointments as needed
