Qualifications

• BSN preferred and CCM certification viewed favorably.
• Three years or more related experience and/or training.
• RN license in state(s) where cases are managed; valid driver’s license.

Responsabilités

• Identify, assess, plan, coordinate, implement, monitor and evaluate services.
• Collaborate with client, family, physician and health-care team.
• Request authorization for services and release of information.
• Develop care plans with achievable short- and long-term goals.
• Coordinate services and resources; monitor progress and adjust as needed.
• Maintain documentation of outcomes and communicate findings.
