Job summary

System Support at Atrium Medical Center is seeking a Chart Completion Tech to support Health Information Management Services. You will prep, scan, and validate paper documents into the scanning system and ensure accurate chart completion.

You will release information for continuity of care requests and monitor incomplete charts, ensuring HIPAA compliance and adherence to JC/participation guidelines. Prior health information experience and basic Word/Excel skills are preferred.

Qualifications

  • Must have completed a course in medical terminology or be enrolled currently.
  • Minimum 1–3 years of job-related experience in Health Information or related medical field preferred.
  • Ability to read and write to review and maintain patient medical record charts; perform basic calculations.

Responsibilities

  • Prepare, scan, and validate paper documents into the optical scanning system for chart completion.
  • Scan and validate loose reports for the department.
  • Release information for continuity of care requests in compliance with HIPAA.
  • Monitor incomplete charts and follow up with physicians to ensure timely completion.
  • Assist with government audits and specialized chart reviews (as applicable).
  • Follow JC guidelines and all federal/state regulations; perform other duties as assigned.

Position: Chart Completion TechDepartment: Health Info MGMT ServicesFacility: Atrium Medical CenterUnder general supervisor, preps, scans, and validates all types of paper documents into optical scanning system to obtain chart completion. Scans & validates all loose reports that are sent to the department. Responsible for analyzing medical records for deficiencies in chart documentation. Responsible for release of information for continuity of care requests. Assures that guidelines of HIPAA are followed to reduce the hospital’s potential liability in releasing records or protected health information. Responsible for processing request for government audit and specialized chart reviews (UVMC only). Responsible for monitoring incomplete charts and follow up with physicians. Responsible for following guidelines of JC and Conditions of Participations and all federal and state. Responsible to perform other duties as assigned and needed.EducationMinimum Level of Education Required: High School completion / GEDLicensure/Certification/RegistrationMedical TerminologyExperienceMinimum Level of Experience Required: 1 - 3 years of job-related experiencePrior job title or occupational experience: Medical OfficePreferred experience: Health Information or related medical field, Knowledge of Word & Excel.Other experience requirements: Must have completed a course in medical terminology or be enrolled in a class at the present time.Knowledge/SkillsAbility to read and write in order to review and maintain patient medical record charts, and perform basic mathematic calculations as well as ability to operate standard office equipment as a level normally acquired through completion of high school.Must be able to work independently in a medical office setting.Problem-solving abilities and basic analytical skills necessary.Must possess excellent organizational skills.Ability to type accurately at acceptable levels of speed.Strong verbal and written communication skills.Interpersonal skills required to effectively communicate, understand, interact, and respond to patients, physicians, and other personnel when obtaining and supplying routine factual medical record information via telephone or direct personal contacts on a daily basis.Basic analytical skills necessary in order to determine work priorities, assist in problem resolution and recommend methods of improving workflow.
