Job summary

Spectrum Retirement Communities, LLC is seeking a Wellness Nurse in Illinois (South Elgin) for a part-time AM shift. The role supports residents’ health through clinical monitoring and collaboration with nurses, caregivers, and medication staff to ensure regulatory compliance and clinical excellence.

Responsibilities include coordinating admissions, ensuring care per care plans, administering medications, coordinating with providers, documenting care, and staying current with state regulations.

Qualifications

• Working knowledge of state regulations, policies, and procedures for compliance.
• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three years’ experience in assessment and geriatric nursing preferred.
• Compassion and a warm, team-oriented attitude.

Responsibilities

• Assist in coordination of the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Medication and treatment administration per physicians’ orders.
• Coordinate care and services with outside providers including resident physicians.
• Document and report care provided and resident observations, including significant changes.
• Maintain knowledge of state regulations, policies, and procedures.
