Spectrum Retirement Communities is seeking a Wellness Nurse in Illinois to support residents through clinical monitoring and collaboration with care teams. The role emphasizes flexibility, teamwork, and compassionate care within a regulated environment.

PRN schedule, with opportunities to coordinate admissions, administer medications, and report on resident changes, while staying compliant with state regulations and procedures.

Qualifications

• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three years’ experience in assessment and geriatric nursing, preferred.
• A big smile, eagerness to learn, and compassion for our Residents.

Responsibilities

• Assist in coordination of the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Medication and treatment administration and ensure medications are administered per physicians’ orders.
• Assistance with coordinating care and services with other outside providers including resident physicians.
• Document and report care provided and resident observations including unusual or significant changes.
• Maintain a working knowledge of state regulations, policies, and procedures.
