Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to company guidelines.
• Provides professional treatment while using therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.
• Documents patient evaluations, treatment progress notes, and discharge summaries for medical records in a timely manner.
• Collaborates with occupational therapists, physical therapists, physicians and other health-care team members relative to patient treatment programs and progress.
• Demonstrates knowledge to provide patient care appropriate in a SNF setting and administers standardized speech/language assessments and swallowing evaluations.
• Ensures delivery of quality rehab services and maintains compliance with the Compliance Program and Code of Conduct.
