Job summary

Grove Park Healthcare Center in East Orange, NJ seeks a Per Diem Speech-Language Pathologist (SLP) to join our dedicated team. The role focuses on improving residents' communication and swallowing abilities within a supportive, patient-centered setting.

The SLP will conduct assessments, develop individualized care plans, and ensure accurate Section GG data collection. Collaboration with rehab staff and education for residents and families are key aspects of this position.

Qualifications

• State licensure as a Speech-Language Pathologist.
• CPR/BLS certification.
• Experience with Section GG data collection and documentation.
• Strong communication and interpersonal skills.
• Critical thinking and problem-solving abilities.
• Modality/Vital Stim certification.

Responsibilities

• Assessment and evaluation of residents' speech, language, cognitive, and swallowing abilities.
• Develop individualized intervention plans for communication and swallowing.
• Collect Section GG data for assessments and quality reporting.
• Administer speech therapy interventions and patient education.
• Maintain accurate documentation in EMRs (Net Health/PCC) and POS care delivery.
• Educate residents, families, and care staff on mobility techniques and remote monitoring.
• Collaborate with Physical and Occupational Therapists for holistic care.
