Qualifications

• Must communicate clearly, verbally and in writing.
• Experience with frail or elderly individuals preferred.
• CPR and First Aid training current or to be completed before independent work.
• Valid Chauffeur’s Driver’s license with a clean driving record.
• Ability to operate wheelchair-accessible vans/vehicles safely.
• Ability to read, write, and speak English clearly.
• Must handle information confidentially at all times.
• Strong interpersonal skills and professional demeanor.

Responsibilities

• Provide safe transportation for participants to the Center and appointments.
• Communicate relevant care information to the Interdisciplinary Team.
• Maintain a courteous, compassionate, and supportive experience for participants.
• Assist with wheelchair transfers between home, van, and Center.
