Job summary

WelbeHealth is seeking a Personal Care Assistant to join our day center team in California. You will provide hands-on personal care to participants, assist with activities of daily living, and help integrate care planning as part of the interdisciplinary IDT.

Responsibilities include taking vital signs, assisting with meals, escorting participants to medical or care appointments, supervising day center activities, and maintaining supplies.

Qualifications

• High school diploma or equivalent required.
• CA Food Handlers Certification within 30 days of hire.
• CPR and First Aid certification within 180 days of hire; certification upon hire preferred.
• Must be willing and able to drive to assignment locations.

Responsibilities

• Provide hands-on personal care to WelbeHealth participants, including assisting with ADLs (toileting, bathing, dressing, grooming, laundry) and lifting when transferring.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort when needed, assisting participants with boarding and exiting WelbeHealth transportation vehicles.
• Oversee activity on the day center floor to assure participant safety.
• Assist with meal service, ensuring participants receive the correct meals and assist with feeding when needed.
• Maintain personal care supplies such as incontinence supplies and grooming items.

Education

High school diploma or equivalent

HHA or CNA certification in California

CA Food Handlers Certification

CPR and First Aid certification
