Job summary

WelbeHealth is seeking a Personal Care Assistant to support seniors at our day centers in California. You will provide hands-on personal care, assist with daily activities, and help coordinate care within our interdisciplinary team.

You will measure vital signs, assist with transportation, and ensure participant safety while supporting meal service and activities. Qualifications include a high school diploma and willingness to obtain required certifications within set timelines.

Qualifications

• High school diploma or equivalent required.
• HHA or CNA certification preferred.
• Willingness to drive between centers and to appointments.

Responsibilities

• Provide hands-on personal care assistance with activities of daily living (ADLs) to participants.
• Measure and document participants’ vital signs.
• Act as a medical escort when needed for WelbeHealth transportation.
• Oversee day center safety and activity flow.
• Assist with meal service and feeding when needed.
• Maintain supplies and equipment needed for personal care.
