Job summary

WelbeHealth PACE is seeking a Personal Care Assistant to provide hands-on personal care at our day center and to contribute to the care planning process as part of the interdisciplinary team. You will report to the Day Center Manager and help deliver safe, compassionate care in a lively day center setting.

Responsibilities include assisting with ADLs, escorting participants, documenting vital signs, helping with meals, and ensuring safety and dignity for all participants.

Qualifications

• High school diploma or equivalent required.
• HHA or CNA certification preferred in California.
• CPR and First Aid certifications recommended.

Responsibilities

• Provide hands-on personal care assistance with ADLs such as escorting, toileting, bathing, dressing, grooming, laundry, and safe lifting.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort when needed for transportation to appointments.
• Oversee activity on the day center floor to assure participant safety.
• Assist with meal service and feeding when needed.
• Maintain personal care supplies.

Education
