Job summary

WelbeHealth in Pasadena, CA is seeking a Personal Care Assistant to support participants at our PACE day center. You will provide hands-on personal care, assist with activities of daily living, and help with meals and safety on the day center floor.

You will document vital signs, provide medical escort when needed, and collaborate with the interdisciplinary team to support care planning. A high school diploma is required with relevant certifications preferred.

Qualifications

• High school diploma or equivalent is required.
• HHA or CNA certification is preferred.
• Minimum of one year of experience providing personal care to seniors.
• CA Food Handlers Certification within 30 days of hire.
• CPR and First Aid certification within 180 days of hire (or at hire preferred).

Responsibilities

• Provide hands-on personal care assistance with activities of daily living (ADLs).
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort when needed for WelbeHealth transportation.
• Oversee day center floor to assure participant safety.
• Assist with meal service and feeding when needed.
• Maintain personal care supplies and grooming items.
