Job summary

Monarch Communities is seeking a Care Manager (per diem) to support residents with daily living activities in accordance with each resident’s care plan. The role is non-exempt and hourly, with a schedule across multiple shifts, including mornings, afternoons, and nights.

The ideal candidate holds a CNA or CHHA certification and has experience with the elderly, with dementia experience preferred. Per diem benefits include PTO, flexible scheduling, on-the-job training, and free parking.

Qualifications

• Certified Nursing Assistant or Certified Home Health Aide certification is required.
• Experience with elderly residents preferred, dementia experience helpful.

Responsibilities

• Provide resident care including bathing, dressing, toileting, and feeding as per care plan.
• Maintain safety, comfort, dignity, and confidentiality of residents.
• Escort residents to meals and activities.
• Ensure residents are clean, neatly dressed, and fed during shifts.
• Document all services provided during your shift.
• Report any change of condition immediately to supervisor.
