Job summary

Bluegrass Care Navigators\' PACE Center is seeking a Part-time CNA to provide direct personal care and support to participants aged 55+ in Kentucky.

Under RN supervision, you’ll assist with activities of daily living, promote safety, document care, and help with transportation as needed. The role emphasizes compassionate engagement, adherence to care plans, and collaboration with the interdisciplinary team.

Qualifications

• High school diploma or GED required.
• Must have Kentucky SRNA certification.
• Completion of an approved CNA training program is required.
• Food handling permit required on hire and CPR certified.

Responsibilities

• Provide direct personal care and supportive services to PACE participants.
• Assist with activities of daily living and promote safety and independence.
• Support engagement in center-based activities and maintain a clean, safe environment.
• Document care accurately and in compliance with regulatory requirements.
• Observe and report changes in participant conditions to the RN.
• Assist with participant transportation as needed.
• Collaborate with the Interdisciplinary Team (IDT) and maintain effective communication.
