Job summary

BAYADA Home Health Care is seeking a Full Time or Part Time Registered Nurse (RN) to join our home health visit team in Harrisburg, Pennsylvania. You will provide skilled nursing and rehabilitative care to clients at home, enabling them to stay in familiar surroundings with compassionate, reliable support.

You’ll work with a multidisciplinary team, perform head-to-toe assessments, manage acute and post-acute conditions, and coordinate care while maintaining documentation and safety.

Qualifications

• Current PA nursing license in good standing.
• Graduation from an accredited nursing program as indicated by transcript or diploma.
• At least one year of hands-on medical experience with adults/geriatric patients in relevant settings.

Responsibilities

• Perform head-to-toe assessments and identify changes in patient condition.
• Manage acute and post-acute care needs in the home setting.
• Triage and independently handle urgent clinical situations.
• Conduct medication reconciliation, teaching, and safety monitoring.
• Perform wound assessment, care, and documentation.
