Qualifications

• Provide acute and post-acute home health care (CHF, COPD, wound care, IV therapy).
• Triage and manage urgent clinical situations in patients’ homes.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Document wound care and overall patient status accurately.
