Job summary

BAYADA Home Health Care in New Hampshire is seeking a full-time Registered Nurse (RN) to join our home health visit teams. You will provide skilled nursing and rehabilitative care to clients in their homes, enabling them to remain at home with compassionate, high-quality service.

Ideal candidates hold an NH nursing license, have acute or geriatric experience, and can manage a dynamic visit schedule while prioritizing patient safety and comfort.

Qualifications

• Current nursing license in good standing in New Hampshire.
• Graduation from an accredited nursing program with transcript or diploma.
• At least one year of adult/geriatric clinical experience in acute care, home health, or similar settings.

Responsibilities

• Provide skilled nursing and rehabilitative care to homebound clients.
• Perform head-to-toe assessments and identify changes in condition.
• Triage and manage urgent clinical situations in the home setting.
• Conduct medication reconciliation, teaching, and safety monitoring.
• Document care comprehensively and coordinate with the care team.
