Job summary

Venture I Inc. seeks a compassionate Registered Nurse (RN) to join our Home Health and Hospice team in Cleveland, TX. The RN will deliver skilled nursing care in patients’ homes or hospice settings, ensuring safety, comfort, and quality outcomes.

The ideal candidate holds a Texas RN license (or compact state), BSN/ADN, CPR/BLS certification, and reliable transportation. Experience in home health or hospice is preferred; on-call rotation may be required.

Qualifications

Current Texas RN license (or compact state)
BSN or ADN from an accredited program
CPR (BLS) certification required
Valid driver’s license, auto insurance, reliable transportation
1+ year clinical nursing experience; home health/hospice preferred

Responsibilities

Conduct initial and ongoing patient assessments in home health/hospice settings
Develop individualized care plans with physicians, patients, and families
Administer medications, IV therapies, wound care, and skilled nursing procedures
Monitor patient progress and report changes to care team
Educate patients and families on disease management and home care
Coordinate care with therapists, social workers, and aides
Maintain accurate EMR documentation and records
Participate in on-call rotations as assigned
Uphold HIPAA and agency policies
Provide compassionate end-of-life support in hospice
