Job summary

Harmony Home Health and Hospice, LLC. is seeking a Registered Nurse to join our team in Clearfield, Utah. You will create and manage individualized care plans, working closely with patients and families to ensure compassionate, high-quality home health care.

Qualifications

• Currently licensed Registered Nurse (RN) in state of employment.
• Previous experience as a Registered Nurse is preferred.
• Excellent interpersonal and client care skills.
• Valid driver’s license.
• First aid/CPR certification required.

Responsibilities

• Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans.
• Administer medications and monitor patients for side effects or reactions.
• Document changes in patient conditions and report to supervisor.
• Maintain accurate records of patient care and discuss observations with the supervisor.
