Job summary

Harmony Home Health and Hospice, LLC. seeks a dedicated Registered Nurse (RN) to join our hospice team in Salt Lake City. You will create and manage individualized care plans, assess patient needs with precision, and communicate effectively with families and the interdisciplinary team.

The role emphasizes compassionate end-of-life care, flexible scheduling, and maintaining high standards of patient comfort and safety. A valid RN license is required, with hospice experience preferred.

Qualifications

• Graduate of an accredited nursing program with RN licensure.
• Experience in hospice or end-of-life care is preferred.
• Ability to observe, assess and document patient needs accurately.
• Strong communication with patients, families and IDT.
• CPR certification and valid driver's license required.

Responsibilities

• Completes comprehensive patient assessments and ongoing monitoring.
• Develops and updates patient care plans with family involvement.
• Coordinates with the interdisciplinary team to ensure appropriate care.
• Delivers hospice nursing care and administers prescribed treatments.
• Educates families on care needs, symptoms, and comfort measures.
