Job summary

Harmony Home Health Service in Clearfield, UT is seeking a registered nurse to plan, organize and direct hospice care in patients' homes. The role emphasizes community health education and collaboration with families.

Qualifications

Graduate of an accredited school of nursing.
One year nursing experience recommended; hospice care experience recommended.
Current licensure in State, CPR certification and valid driver’s license.
Responsible for supervising hospice aides.
Excellent observation, verbal and written communication skills; problem solving.

Responsibilities

Completes an initial and ongoing comprehensive assessment of hospice needs.
Applies criteria for admission and re-certification to hospice care.
Assess caregiver ability and coordination for home care.
Develops and revises plan of care with patient/family involvement.
Provides specialized hospice nursing care and communicates with the interdisciplinary team.
Initiates plan of care and revises as patient needs change.
Provides health instructions and supports activities of daily living.
Participates in on‑call duties and ensures equipment and services are available.
