Job summary

Harmony Home Health and Hospice, LLC. in Murray, UT is seeking a Registered Nurse to join our team. You will create and manage a plan of care for individual patients, observing and communicating to support the patient’s best interests with attention to detail.

The role requires a 4 year degree, RN license, CPR certification, and the ability to collaborate with an interdisciplinary team to assess needs, initiate plans of care, and provide education and compassionate care at the end of life.

Qualifications

• RN must observe/assess patient needs with strong clinical judgement.
• Excellent communication with patients, families and IDT.
• Ability to develop and manage individualized care plans.
• Strong problem-solving and critical-thinking skills.

Responsibilities

• Completes initial, comprehensive nursing assessment and ongoing evaluation per hospice needs.
• Implements admission criteria and levels of care.
• Develops and revises plan of care with patient/family input.
• Provides care coordination and education to patient and family.
