Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) for home health visits in Pennsylvania. Roles include per diem, part-time, or full-time opportunities with flexible schedules to serve adult and geriatric patients.

Responsibilities include following care plans, administering medications, wound care, and coordinating with the Clinical or Client Services Manager. Travel to cases is required as assigned.

Qualifications

• Current PA RN license.
• Minimum 1 year clinical experience; home care preferred.
• Graduate from an accredited nursing program.
• Ability to work independently and manage time.
• Strong interpersonal skills.
• Proficient with EMR systems.
• Ability to travel to cases.

Responsibilities

• Follow a designated care plan in accordance with patient needs.
• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments and procedures.
• Monitor clients' conditions and report changes to supervisor.
• Follow up with doctors' orders and document appropriately.
• Perform client assessments as necessary.
• Case management and coordination.
• Document observations and care activities using a tablet.
