Job summary

Ambercare Hospice in Deming, NM is seeking a compassionate PRN Registered Nurse Case Manager to join our team. You will deliver expert clinical care and support to patients and families, helping ensure comfort, dignity, and quality of life.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Maintains a current CPR certification.
• Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and general public.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assist with patient and family caseload, including assessment, eligibility, admission processing, planning, implementation and evaluation of the nursing process.
• Obtain data from patient’s physical, psychological, social and spiritual factors affecting needs and initiate intervention by the interdisciplinary team.
• Coordinate care by initiating communication with the attending physician and other agencies.
• Provide visits to reassess the patient’s needs and update the plan of care.
• Perform nursing procedures as directed by the attending physician.
