Job summary

Ambercare Hospice, part of the Addus HomeCare family, seeks a compassionate PRN Registered Nurse Case Manager to deliver expert clinical care and support for patients and families in Truth or Consequences, NM. You will manage a patient caseload, coordinate with the interdisciplinary team, and ensure comfort and quality of life.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications required.
• Valid driver’s license and proof of insurance is required.
• Knowledge of quality assessment and assurance procedures.

Responsibilities

• Assume primary responsibility for patient and family caseload across nursing process stages.
• Obtain data from patient factors and coordinate with the interdisciplinary team.
• Coordinate care with attending physician and other agencies for optimal care.
• Provide regular home visits to reassess needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts and care plans timely.
• Supervise and document delivery of care by Home Health Aide per guidelines.
