Job summary

Ambercare Hospice is seeking a compassionate Registered Nurse Case Manager to join our team in Belen, NM. You will manage patient care and support families during critical moments, working within a friendly, supportive environment.

Qualifications

• Must possess current CPR and First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload.
• Obtain data and assess necessary information from patients.
• Initiate communication with the attending physician and interdisciplinary team.
• Provide regular visits to re-assess patient's needs.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records, charts, and care plans.
