Job summary

Bristol Hospice in Fairfax, VA is seeking an RN Case Manager to provide compassionate, whole-person hospice care and advocate for patients and families. This full-time role focuses on care coordination, assessments, and timely communication with interdisciplinary teams to ensure comfort and dignity throughout the care journey.

You will administer physician-ordered medications, monitor vital signs, and educate families on care techniques while maintaining detailed clinical notes.

Qualifications

• Active Registered Nurse license in the state you will work in (Virginia).
• Compassionate, whole-person care and patient advocacy.
• Experience coordinating care for patients and families.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support to patients and families to ensure comfort and dignity.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor vitals and document changes in patient condition.
• Educate families on care techniques and symptom management.
