Bristol Hospice is seeking an RN Case Manager in Fort Worth, TX to deliver compassionate, holistic hospice care. You will develop care plans, support patients and families, and collaborate with an interdisciplinary team to uphold comfort, dignity, and quality of life on this journey.

Qualifications

• Active RN license in good standing in the state you work.
• Current CPR certification.
• Strong observational and communication skills, written and verbal.
• Valid driver’s license and reliable transportation.
• Willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Provide one-on-one support to patients and families to ensure comfort and dignity.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor vital signs and document changes.
• Administer physician-ordered medications and treatments with empathy.
• Educate families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
