Job summary

Rappahannock Area Community Services Board seeks compassionate Direct Support Professionals to help adults with developmental disabilities in their homes and community activities, following individualized plans of care.

You will support personal care, meals, transportation, appointments, and medication administration, document progress, and coordinate with families and the care team. A Virginia driver's license and high school diploma are required.

Qualifications

• Requires effective communication in writing and speech.
• Experience with documentation of participant records preferred.
• Good physical condition and ability to assist participants with limited mobility.

Responsibilities

• Provide support with personal care, household tasks, meal planning, and social activities.
• Assist residents with physician appointments and transportation.
• Administer and document medications.
• Document daily progress notes and report needs to the care team.
• Greet visitors, supervise visitation, and ensure home safety.
• Follow each resident's care plan in collaboration with family and providers.
• Provide awake overnight supervision as needed.
