Job summary

HumanGood is seeking a Social Services Coordinator in New Jersey to organize, implement and evaluate our Social Services program to meet residents' psycho-social needs. This includes resident assessment and linking residents to supportive services to help them continue living independently in the community.

The role is full-time, Monday–Friday, 9:00 AM–4:00 PM, with a Bachelor's degree in social work or related field, 1+ year of case management, and 2+ years of social services experience;

Qualifications

• 1+ years of case management experience.
• 2+ years of social services experience, preferably in senior care.
• Bachelor’s Degree in social work or related field required.

Responsibilities

• Organize, implement and evaluate the Social Services program to meet psycho-social needs.
• Perform resident assessments and maintain records to support independence.
• Link residents to supportive services and ensure HIPAA privacy compliance.
• Collaborate with the community team to promote a positive living environment.
