Job summary

Grace Management Service is seeking a Personal Care Assistant at our Wayne Manor facility in Wooster, Ohio. The role requires a High School Diploma or GED and the ability to pass a Nurse Aide Certification class.

Responsibilities include following policies, assisting residents with mobility and dining, documenting activities, and reporting changes to supervisors. Must be able to lift up to 75 pounds and work in a healthcare setting.

Qualifications

• High School Diploma or GED preferred.
• Ability to pass Nurse Aide Certification class.

Responsibilities

• Follow established performance standards and policies.
• Identify resident problems and report to supervisor.
• Assist residents with getting in/out of chairs or wheelchairs.
• Assist with passing water pitchers and trays in dining room.
• Remind residents of meal times and escort to dining room.
• Assist with keeping rooms neat.
• Provide individual attention via walks, group activities, or one-on-one visits.
• Document as required.
• Report changes in residents’ conditions to supervisor or nurse.
• Listen to resident and family complaints and report information.
• Stock laundry and linens.
• Perform other duties as directed by the Administrator or Director of Nursing.
