Job summary

Bethesda Health is seeking an LPN to provide essential nursing care in a facility serving geriatric residents. You will perform admissions assessments, update care plans, and coordinate with the IDT to ensure high-quality resident care.

The role requires an active LPN license in Missouri, a willingness to obtain IV and CPR/AED certifications if needed, and a strong focus on communication, critical thinking, and organizational skills.

Qualifications

• Education from an accredited school of nursing resulting in Licensed Practical Nurse (LPN)
• Long-term care/Rehab/Assisted Living experience preferred
• Current license in the state as an LPN
• IV Certification required or willing to obtain within six months
• Current CPR/AED Certification required or willing to obtain within six months
• Able to assess basic nursing care needs of geriatric residents
• Excellent communication skills with residents, visitors and staff
• Excellent time management and organizational skills
• Must possess critical thinking skills
• May be assigned other duties as appropriate

Responsibilities

• Provide basic nursing care by assisting in resident assessments on admit and ongoing basis
• Communicate significant changes to the interdisciplinary team
• Accurately document assessments
• Participate in the care planning process and update plans as interventions change
• Carry out nursing interventions per the care plan including medication administration
• Administer IV fluids only if IV certified
• Document interventions timely and accurately
• Use SBARR to notify physician timely
• Maintain composure during crises and ensure safe, professional care
• Monitor care plan delivery and use QAPI for problem resolution
• Assist with ADLs and respond to call lights
• Complete at least twelve hours of nursing continuing education
• Stay current with Bethesda policies and guidelines
• May be assigned other duties as appropriate
