LCMC Health is seeking a dedicated nurse to lead and coordinate care touchpoints for patients in a collaborative, multidisciplinary environment. The role emphasizes safety, continuity of care, and exceptional patient experiences.

You will monitor patient conditions, engage with families, and ensure timely communication. Opportunities to educate patients and families and contribute to a supportive team culture are emphasized.

Qualifications

• Current nursing license to practice is required.
• BLS certification is required.
• CPI Level 1 certification is required.
• PALs certification required (CHNOLA only).

Responsabilités

• Take the lead: prioritizing, coordinating, and delegating care touchpoints for patients.
• Collaborate with multidisciplinary team to ensure direct patient care.
• Maintain and advocate a safe environment for everyone.
• Monitor and engage with patients, families, and care team regarding events, changes, or questions.
• Prevent and treat wounds; monitor infection control and environment of care requirements.
• Ensure continuity of care, efficient throughput, and effective communication for an exceptional patient experience.
• Consult with supervisor when unable to complete tasks promptly or safely.
• Initiate patient and family education using appropriate resources and continuum of care services.
• Contribute to the team by assisting others without being asked.

Formation

RN license (Louisiana)

BLS certification

CPI Level 1

PALs certification (CHNOLA only)
