Job summary

Extended Care LLC is seeking a dedicated LPN to deliver patient-centered nursing care in a supportive facility. You will coordinate with a multidisciplinary team, supervise nursing assistants, and ensure accurate documentation while maintaining high standards of safety and confidentiality.

Qualifications

• Valid Nursing License as required by the state.
• Ability to work every other weekend.
• Compliance with the state Health Care Worker Background Check requirements.

• Ability to perform diverse care tasks in a multidisciplinary team.
• Familiarity with health, safety, and sanitation standards.

Responsibilities

• Direct day-to-day activities of the nursing assistants.
• Complete required resident records at admission, discharge, and transfer.
• Prepare and administer medications as ordered by a physician.
• Make daily rounds and manage tasks in a fast-paced environment.
