The Connection is seeking a Case Manager 2 in Hartford, CT, to provide client-centered home and community-based case management for Rapid Re-Housing clients aged 18-24. You will work under the Program Manager, coordinating with landlords and funders to ensure rents and housing paperwork comply with program requirements.

Responsibilities include maintaining client files, participating in audits, and updating data in CAMIS HMIS, contributing to timely monthly reports and quality assurance.

Qualifikationen

• Associate’s Degree or higher required.
• 2+ years related experience in direct service/case management.
• Valid Connecticut Driver’s License required.

Aufgaben

• Provide client-centered home and community-based case management to Rapid Re-Housing clients in the Hartford area.
• Coordinate with landlords, office and funders to ensure rents, leases and housing paperwork are maintained per program requirements.
• Maintain client files per agency and funder standards.
• Participate in audits by TCI and HUD.
• Maintain up-to-date data in CAMIS On Line and HMIS.
• Prepare monthly reports and ensure quality assurance of services.
